Your Care Journey

What to expect, step by step

From the first phone call through the Care Plan and ongoing support, here is what the process looks like.

What to expect

Choosing care for someone you love is a big decision. Knowing what happens at each stage, from your first call through months of support, makes it easier to move forward with confidence.

From first call to settled-in care

The five stages of getting started

01

Your first conversation

The process starts with a free phone consultation. We learn about your loved one’s needs and answer your initial questions.

02

The in-home assessment

The next step after the phone consultation is an in-home assessment. We look at daily routines, mobility, the home environment, preferences, capabilities, and the assistance being considered. The assessment is a paid professional service, separate from the free phone consultation.

03

A Care Plan shaped around the assessment

Using what we learn, we build a personalized Care Plan around your loved one's needs, capabilities, preferences, and daily routines. It can include daily living assistance, companionship, mobility assistance, and other non-medical support within our scope.

04

Caregiver assignment

We consider care needs, schedule, preferences, and personality when making assignments. Every Caregiver is a W-2 employee who is background checked, fingerprinted, trained, insured, and supervised by our team.

05

Care is coordinated

Once the Care Plan, schedule, and pricing are confirmed, care can be coordinated. Our team stays involved as everyone gets familiar with the routine and the Care Plan.

The first weeks

Settling into a rhythm

The early weeks are about learning the routine and building comfort. Our team stays in communication and can review the Care Plan when needs, preferences, or circumstances change.

If something about the schedule, Care Plan, or Caregiver assignment needs attention, let us know. We will review the concern and discuss the available next steps with you.

Over time

How care grows with your loved one

01

We stay in communication

Our team stays in contact about scheduling, Care Plan needs, and changes the family brings to our attention.

02

A Care Plan that keeps up

Your Care Plan can be reviewed as needs, capabilities, and circumstances change, so the plan can be updated when appropriate.

03

Support can be reviewed

If the amount or type of assistance needs to change, the Care Plan and schedule can be reviewed. Changes are coordinated around the updated needs and schedule.

04

The Care Plan stays central

When schedules or Caregiver assignments change, the Care Plan gives the team a consistent guide to your loved one’s needs, preferences, and routines.
